Correction for Venetoclax restriction

The “Initial treatment – Extension of dose titration” restriction (Treatment-of Code 8565) for Venetoclax PBS Item codes 11624T and 11648C appeared incorrectly in the 1 March 2019 Schedule of Pharmaceutical Benefits.
The conjunction between Clinical criteria used “OR” when it should have been “AND”.
The text has been updated and appears correctly in the 1 April 2019 Schedule of Pharmaceutical Benefits.
